Register online

We are using an NHS online service called Register with a GP surgery that makes it easy to register with this GP surgery.

Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number.

The service is designed and run by the NHS, so your personal information is safe. It reduces our administrative workload and makes it easier for you to register.

Paper forms are still available if you need one.

Your NHS number is a 10 digit number, like 485 777 3456. You do not need to know your NHS number to use NHS services, but it can be useful to have it.

When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.

Register at our GP surgery with a paper form

Acceptance on to the list is as at the discretion of the Doctor. We will ask you for 2 forms to prove identity (1 has to show your photo), and proof of your address; though being homeless will not prevent you from being accepted onto the practice list (Please refer to our registration policy below and why we ask for this). Please note that your medical records may take several weeks to arrive at the Practice.

Medical treatment is available from the date of registration. Please contact reception for further information.

Accountable GP – Named GP Policy – Baddeley Green Surgery

There are a number of reasons why you may not be able to register with us, for example you may live too far away. If this is the case simply choose another GP in your local area.

The National Care Record Service (NCRS)

The National Care Record Service (NCRS) is the way in which health professionals now access the Summary Care Record (SCR). The SCR has not changed, it is just the way SCRs are accessed that has changed.
