If you are about to turn 16 or have just turned 16 we now need your details and not your parents, as you are classed as an adult by the NHS, please contact the surgery to update your details. We can accept written permission from yourself to talk to your parents about medical issues but this needs to be signed by yourself and dropped into the surgery.
